The Services for Outpatient Addiction and Recovery (SOAR) RN case manager demonstrates leadership in delivering efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication, and coordination of resources. The RN case manager works in partnership with the addiction psychiatry team and additional SOAR team members to manage care for a panel of patients with a primary diagnosis of substance use disorder (SUD).
