The SARRTP Admissions Coordinator demonstrates exceptional administrative & case management skills. You will be required to independently assess new referrals & determine appropriateness for admission to the SARRTP unit. Mostly, veterans referred to SARRTP are psychiatrically complex & require a high level of expertise in understanding & treating co-occurring SUDs & other mental health problems. The admissions Coordinator will also facilitate referrals to other treatment services as needed. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Grade Determinations: Experience and Education: One year of experience equivalent to the GS-11 grade level. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Oversight & Management of the Admission Process: Receives new referral requests via electronic record or direct contact from a variety of internal & external referral sources requiring excellent interpersonal & liaison skill. Coordinates with local services, including information & referral for additional resources from other VA, government, & community-based programs. Collaborates with & develops VA & community stakeholder relationships. Serves as the primary liaison to the RRTP referral network in VISN 19 & across the National RRTP system. Evaluates substance use disorder treatment needs of new referrals, including assessment of American Society of Addiction Medicine (ASAM) criteria. Evaluates other mental health treatment needs through chart review & direct interaction with referral sources. Coordinates daily & as needed with SARRTP Program Manager & treatment team regarding new candidates & their appropriateness for admission. Facilitates referral to other treatment providers or programs for individuals not deemed appropriate for admission to SARRTP. Facilitates admission to SARRTP for persons assessed as clinically appropriate. Provides ongoing, active case management for persons awaiting treatment with weekly contact, assessment of stability, safety, & suicide risk. Accesses resources as needed to restore stability & safety for these Veterans. Provides program orientation for newly admitted residents. Provides coverage for other SARRTP mental health therapists as needed. Provides coverage for SARRTP Program Manager as needed. Admissions Coordinator will maintain data related to admissions & discharges to comply with VA & medical center policy & procedures, the Joint Commission, & the Commission on Accreditation of Rehabilitation Facilities (CARF). Participates in workgroup and/or committee assignments, as deemed appropriate by the SARRTP Program Manager. Communicates effectively, both orally & in writing, with people from varied backgrounds. Consistently communicates & treats customers (veterans, their representatives, visitors, & all VA staff) in a courteous, tactful, & respectful manner. Provides the customer with consistent information according to established policies & procedures & handles conflict & problems in dealing with the consumer constructively & appropriately. Performs other related administrative duties as assigned. Direct Patient Care: Provides a full range of advance practice social work services, including individual, family, & group psychotherapies, discharge planning, case management & other services as appropriate. Independently conducts psychosocial assessments & formulates individualized treatment plans. Skillfully identifies & responds to unsafe situations, facilitates crisis intervention assistance, mediates conflict resolution, and/or makes necessary referrals to ensure a safe outcome. Meets individually with each veteran assigned to their caseload to provide psychotherapy services, evaluate treatment plan progress, & implement changes as needed. Documents appropriately in the electronic health record. Delivers Evidence Based Therapies (EBT) for SUDs & PTSD, i.e., Cognitive-Behavioral Therapy for SUD, & Prolonged Exposure & Cognitive Processing Therapy for PTSD as indicated. Coordinates treatment & communicates with care lines to facilitate continuity of care for veterans. Provides clinical supervision to students, interns, & less experienced staff in a way that promotes professional development & excellent service delivery. Evaluates their practice on an on-going basis through participation in professional peer review, case conferences, research studies, or other organized means.
