This position is located at the Jesse Brown VA Medical Center (578) - a Level 1b Complexity VHA Facility in the Whole Health Program. The Social Services Assistant (Whole Health Partner) functions as an interdisciplinary team member, coordinating and connecting Veterans to Whole Health resources and programs. The position requires knowledge of internal and external resources to support the Veteran journey through the Whole Health System.
