To qualify for this position, applicants must meet all requirements within 30 days of the closing date of this announcement. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-7 position you must have served 52 weeks at the GS-5. For a GS-9 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. Basic - Individual Occupational Requirement: Bachelor's degree or 4 years of education above high school leading to a bachelor's degree in behavioral or social science or a related discipline appropriate to this position. OR Combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. OR Four (4) years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. In addition to the Individual Occupational Requirement as stated above the following is the experience or education requirement to qualify for this position at the GS-7/9 level. Specialized Experience GS-7: One (1) full year of specialized experience that equipped the applicant with the particular knowledge, skills, and abilities to perform successfully the duties of the Social Science Program Coordinator position, and that is in or related to the work of the position to be filled. To be creditable this experience must have been equivalent to at least the GS-5 level in federal service to qualify at the GS-7 grade level. Specialized experience for this position would consist of: Knowledge of health coaching structure, process, skills and techniques. Knowledge of ethical code and rules of conduct for health coaches. Knowledge of current research literature related to health and wellness, including behaviors that promote wellness and those that are detrimental. Interpersonal skills to present recommendations that improve patient services. Ability to use automated systems and interpersonal and communications skills. OR Substitution of Education for Specialized Experience GS-7: One (1) full year of graduate level education or superior academic achievement. Such education must demonstrate the knowledge, skills, and abilities necessary to do the work of the position and been obtained at an accredited college or university. Superior Academic Achievement is based on class standing, grade-point average, or honor society membership. Class Standing - Applicants must be in the upper third of the graduating class in the college, university or major subdivision, such as the College of Liberal Arts or the School of Business Administration, based on completed courses. Grade-point Average - Applicants must have a grade point average of 3.0 or higher out of a possible 4.0 as recorded on their official transcript, or as computed based on 4 years of education, or as computed based on courses completed during the final 2 years of curriculum; or 3.5 or higher out of a possible 4.0 based on the average of the required courses completed in the major field or the required courses in the major filed completed during the final 2 years of the curriculum. Election to membership in a National Scholastic Honor Society - Applicants can be considered eligible based on membership in one of the national scholastic honor societies listed by the Association of College Honor Societies. OR Substitution of Education for Specialized Experience GS-9: Must possess two (2) years of progressively higher level graduate education leading to a master's degree or master's or equivalent graduate degree. Such education must demonstrate the knowledge, skills, and abilities necessary to do the work of the position and been obtained at an accredited college or university. Zablocki VA Medical Center in Milwaukee, WI and is part of the VA's Great Lakes Health Care System in the Veteran's Integrated Service Network (VISN 12).This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff. Major Duties: Works with individuals and groups throughout the facility in a client-centered process to facilitate and empower the client to develop and achieve self-determined goals related to health and wellness. Coaches support clients in mobilizing internal strengths and external resources and develops self-management strategies for making sustainable, healthy lifestyle behavior changes and more effectively managing chronic disease. Assist Veterans with weight management, smoking cessation, addressing problematic areas in the eight components of proactive health and wellbeing, and adherence to chronic disease such as diabetes, congestive heart failure and chronic pain. Contacts eligible participants for enrollment and explain the health coaching process. Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Actively guides Veterans in the exploration of core elements of personal health planning through individual and group based programs. Develops treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic disease. Maintains education and knowledge regarding lifestyle (habits and behaviors) and clinical outcomes, including behaviors that improve Veterans' health and well-being and those related to poor clinical outcomes. Coordinates with others on the Veteran's health care team to ensure optimal patient care. Develops information for Veterans about the Whole Health Group and Individual Programs.
