Join Providence Hospice of Snohomish County as a full-time Palliative Care nurse practitioner. This agency started a home-based Palliative Care program one year ago where the nurse practitioner is the main provider. As the provider for this program, you will make home visits to the Palliative Care patients with follow up visits being done in person or virtually, as appropriate.
