The social worker is assigned to the Community Living Center (CLC) at the St. Cloud VA Health Care System. Social Worker is responsible for providing social work services to the Veteran residents of this area. The incumbent is a member of an interdisciplinary team, and is primarily responsible for completing biopsychosocial history and assessments, completing the psychosocial portions of the care plan and the Minimum Data Set (MDS). Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
