St. Peters Health Partners is seeking a full-time Nurse Practitioner or Physician Assistant to join Palliative Care Partners, a service line across St. Peter's Health Partners supporting patients and their families who are dealing with serious illness, located in the Capital District, New York.The selected Advanced Practice Provider (APP) will provide palliative care services in the community / home settings including home visits, tele-medicine, on-call rotation, and collaboration with the interdisciplinary team.This role will work closely with an interdisciplinary team including a physician, Social Worker, and Spiritual Care support to provide comprehensive care to patients with advancing illness primarily in the community setting.
