Our Oncology Chemotherapy Infusion unit compassionately cares for cancer patients who are receiving chemotherapy through infusion (IV) therapy, where therapies and medications are administered directly into our patientsâ™ bloodstreams. We are proud to be an accredited Chest Pain Center with an advanced telemetry monitoring system, a Primary Stroke Center, and a Level III Trauma Center.

Your day.
As an Oncology RN Patient Navigator, you need to know how to:
- Provide guidance to the patient through the care process and plan of care. Establish early contact with cancer patients to identify and eliminate barriers to timely diagnosis and treatment. Serve as a clinical resource with expertise in oncology care management in reinforcing education with patients and families.
- Utilize the nursing process to provide coordinated patient care by actively collaborating with other members of the healthcare team and case managing patients. Assess clinical, emotional, spiritual, psychosocial, financial and other patient needs.
- Provide direct patient and family education, as well as community outreach, education, and screening. Coordinate discharge planning, development of a post-acute care plan, and coordination of post-acute services. Refer patients to appropriate clinical trials and supportive care services, as well as direct patients and families to available community resources.
- Direct follow-up on patientsâ™ progress relative to their post-acute plan. Monitor and report on outcome measures.
- Question and prevent duplication, fragmentation, or use of unnecessary resources. Question tests/procedures for which the purpose is unclear.
- Track and collect data on patients including demographics, length of stay, and reason for extended stay. Participate in readmission analysis on assigned patients with sending hospital and inclusive of post-acute providers. Provide reports as needed.
- Promote mission, vision, and values of SCL Health, and abide by service behavior standards.
- Perform other duties as assigned.

- Graduate of an accredited school of nursing, required
- Current license to practice as a Registered Nurse in the state of practice or ability to obtain, required
- Minimum of five (5) years of recent nursing experience in an acute care or post-acute care setting, required
- Bachelorâ™s Degree in Nursing (BSN), preferred
- Current OCN certification, preferred
- At least three (3) years of recent nursing experience in Oncology nursing, preferred
- Previous care management experience, preferred
