Rehab and Extended Care Integrated Care Community, Social worker assigned to Home Community Care are responsible for providing direct patient care in the community and via video telehealth as a member of a multidisciplinary treatment team. Serves a variety of patients located either in their own homes, who may be receiving HBPC (home based primary care) or AC (alternative care) services or are living in a supportive setting such as a VA contract nursing home or Medical Foster Homes (MFH).
