| VA Boston Healthcare System Social Worker Services is seeking a licensed Social Worker to serve as a Physical Medicine & Rehabilitation (PM&R) Social Worker and Transplant Social Work Coordinator. The incumbent is stationed at the West Roxbury (WR) Campus, but is expected to travel to all campuses and CBOCs within VA Boston Healthcare System as needed. This position is supervised by the West Roxbury Social Work Supervisor with input from the Chief of PM&R and Transplant Program Team. As the Transplant Social Work Coordinator, the incumbent: Serves as the social worker assigned to the VA Boston National Transplant Center Works collaboratively with VA and community Transplant Center's including new Transplant Program with BWH. This program is mechanical circulatory support and heart transplantation Must have medical Social Work experience and education that demonstrate possession of advanced practice skills and judgment Must have at least two years in an area of specialized social work practice, preferably in transplantation Must be licensed or certified by a state at the advanced practice level which included an ASWB clinical examination Advanced practice transplant social worker independently provides clinical psychosocial and case management services at the advanced practice level to eligible veterans and their families, to meet the biological, psychological, social, and environmental needs. The veteran's health care and biopsychosocial problems & needs are complex and require a high degree of clinical oversight and creative problem solving. Services are provided in the community, at the VA that referred the veteran to transplant, during the pre-transplant and posttransplant periods, and at the time of transplant. The incumbent is required to interact with the patient, family, and significant others, as well as community agencies, numerous treatment teams, and staff members from any VA in the U.S. and its territories. As the PM&R Social Worker, the incumbent services as the primary social worker on the Acute Rehab Inpatient Unit. The Incumbent: Serves as social work program coordinator for the Amputation System of Care and the Comprehensive Integrated Interdisciplinary Rehabilitation Programs Is administratively responsible for a clinic program providing treatment to patients in a major specialty, such as Amputation Care and Rehabilitation and other specialized programs May be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area Manages the operation of the social work program and direct patient care services in the program area Participates in the department QM and CARF committee meetings and develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and/or national use Manages & controls the use of data related to these programs regarding day to day functioning and program evaluation In conjunction with the Chief of Physical Medicine and Rehabilitation, plans program development, organizes and directs administrative, operational, patient and personnel activities for the service Provides clinical care & case management for veterans enrolled in VA BHS Provides social work care and for the Comprehensive Integrated Inpatient Rehabilitation Program (CIIRP) and the VISN 1 Amputee System of Care (ASoC) program Provides care coordination in conjunction with primary care providers and specialty clinics as needed for Boston and VISN patients to meet their complex and time sensitive needs. In additional to coordinating interdisciplinary care within the VA Healthcare system, as the services to veterans with amputations progresses, the incumbent may assist the veteran and their caregiver across the continuum of care including living at home and receiving in home services, outpatient care and institutionalized care Assists with accessing VA and community benefits and caregiver support The incumbent has dual role of lead social worker for the Comprehensive Integrated Interdisciplinary Rehabilitation Program and the Amputation System of Care (ASoC) program. As a result, requires specialized inpatient and outpatient clinical social work competencies and serves as a social work subject matter expert for ASoC on topics of limb loss prevention, coping and community reintegration after limb loss.
