The Social Science Program Coordinator is located at the West Palm Beach (WPB) Domiciliary Residential Rehabilitation Treatment Program (DRRTP) facility. The incumbent serves primarily as the Whole Health Program Coordinator as well as assists with quality improvement and food service duties. Population served are adult Veterans ages 18-75 years and may a variety of serious mental health, alcohol and drug addictions, medical illness, and other psychosocial problems. The incumbent performs duties with a high-level of independence and demonstrates competence regarding Whole Health nutrition and food-service operations under the supervision of the Nutrition Program Manager. The increasing complexity and specialization in whole health nutrition requires that the incumbent have expertise in a wide range of disease entities and possess the skills necessary to provide care appropriate to the whole health journey, specifically through the pathway, well-being, and Whole Health clinical care elements of the Whole Health System. Specific duties of the position may include, but are not limited to the following: Developing, managing, and evaluating whole health programs for Veterans and staff. Providing health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Developing and leading employee customer service training and maintaining a customer service plan for Nutrition and Food Service (NFS). Developing and monitoring processes to maintain safety and sanitation standards that meet all regulatory agencies. Participating in performance improvement activities by assisting in the development of indicators and monitors, data collection and analysis, corrective actions and preparation of required reports. Responsible for conducting Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other outpatient groups as appropriate. Conducting interventions for Veterans using one-on-one, couple, or group modalities in outpatient settings. Conduction outreach activities to inform Veterans and family members of Whole Health programs. Evaluating training needs and establishing necessary training schedules and mandatory/nonmandatory topics. Utilizing VISTA, CPRS, Microsoft office software, intranet/internet and other applicable software for tracking and supporting patient progress.
