Overview The Adolescent Dual Diagnosis program is a unique pediatric and adolescent inpatient hospital program that offers services to teens who are experiencing mental health and chemical dependency challenges. Members of the interdisciplinary team include Medical Doctors, Registered Nurses, Psychiatric Associates, Rehabilitation staff (music therapist, art therapist, recreational therapist), Occupational Therapists, Chemical Dependency Counselors, Chaplains, and an Advance Practice RN with specialization in Child/Adolescent Psychiatric-Mental Health Nursing. In addition to joining a team that is part of one of the largest hospitals based Mental Health and Addiction Services service lines in the country, an individual who works in this program has access to many resources for expanding their clinical expertise. Clinical staff report to the Manager of Clinical Services for the pediatric and adolescent units, who works closely with the programs Medical Director and Staff Council in promoting patient centered care that is effective and efficient. Clinical Treatment Coordinator (CTC) will work a block schedule with a rotating weekend from 11:00am to 7:00pm for a total of 80 hours a pay period. CTC will partner with the physician and interdisciplinary team to navigate the patient/family through stabilization and work toward ongoing care through therapeutic interventions with an emphasis on group therapy.Duties will include assessment on admit to plan for discharge and work with the identified a treatment plan, community re-entry meetings, discharge planning, group, individual, and family therapy. This includes but is not limited to: managing patient/family expectations, responding to the crisis of hospitalization, supporting the discharge and community re-entry needs and monitoring and managing patient length of stay and avoidable days.
